Medical Case Management Services: The medical case management staff interacts with clients in a supportive role to facilitate access to community resources, enhance psychosocial wellness, and foster personal independence and resilience. Clients receive individual psychosocial support services and facilitated group services. Case management services focus on problem-solving in a one-on-one environment. Services are currently focusing on the needs of clients facing the challenges of taking complicated combination drug therapy regimens (for more information, go to adherence page), and assessment of clients in the process of adjusting to newfound health. Practical Support Services: The Center also provides clients with practical support. Services include housing assistance, travel reimbursement for HIV-related medical and psychosocial appointments, and access to complimentary therapies within the community.

  • Assistance in accessing home care

  • Referral to substance abuse services
